Birth/Death Registration

  I. IMPORTANT GENERAL INFORMATION
1) All Birth & Death certificates are issued through the Janma Mrityu Tathya Portal of Govt. of West Bengal since 05th May 2022.

2) Birth or Death certificates can be obtained only if the event of birth or death is registered with the KMC.

3) Under the existing rules any event of the birth or death is to be compulsorily registered within 1 year of its occurrence.

4) Delayed registration beyond one year of occurrence of birth or death can only be done on the basis of orders of Executive / 1stClass Magistrate of the KMC area. If the event of Birth is within 5 years of the Date of Birth then the same will be issued from respective Borough Ex Health Officer. Post 5 years all such certificates will be issued only from KMC, Central Municipal Office Building.

5) Births or deaths occurring in places within the area under KMC jurisdiction can only be registered by the KMC.

6) The entire KMC area is divided into 16 boroughs for administration and the office of the Executive Health Officer of each borough is the Registration Unit for all births occurring within the respective Borough area.

 
  II. PROCEDURE TO OBTAIN BIRTH CERTIFICATES
A. For Birth events occurring in Private Hospitals / Nursing Homes in Kolkata

1) Birth certificate can be obtained from the concerned office of the Borough Ex. Health Officer usually within 4 weeks from the date of birth, subject to the condition that the birth event has been reported in prescribed registration format by the private hospitals / nursing homes within 21 days from the date of occurrence to concerned borough Ex. Health Officer as a mandatory requirement and the private health facility does not have access to Janma Mrityu Tathya Portal of Govt. of West Bengal.
2) For such Private Hospitals / Nursing Homes in Kolkata which have access to Janma Mrityu Tathya Portal of Govt. of West Bengal the requisite data will be input in the portal from the health facility and the same will be forwarded to the concerned Borough Ex. Health Officer for further processing.
3) A link will be sent via SMS to the parents to the mobile number as provided by the Private Hospital/ Nursing Home for incorporation of the Name of the child. Once the same is incorporated another SMS will be forwarded to the same mobile number with a link to download the Birth Certificate.


B. For birth occurring in Govt. Hospitals in Kolkata

1) Birth certificates can be obtained from the concerned Govt Hospital directly through the Janma Mrityu Tathya Portal of Govt. of West Bengal.


C. For Domiciliary birth events:
  IT IS ENCOURAGED TO HAVE ALL BIRTHSIN AN INSTITUTIONAL SETTING FOR THE BENEFIT OF THE MOTHER AND CHILD

1) Domiciliary events of birth shall be reported by the Head of family to the concerned Ward Health Unit for registration within a month but not later than 1 year along with production of medical certificate of birth issued by a Medical Practitioner / Midwife / Traditional Birth Attendant. Birth certificate for such Domiciliary Births will be issued post physical verification by local authority.
2) Birth certificates in respect of domiciliary events not reported within one year can be obtained on delayed registration on the basis of orders of Executive / 1st Class Magistrate of KMC area from the concerned borough Ex. Health Officer [up to 5 years] and KMC, Central Municipal Office Building[beyond 5 years].

 
  III. PROCEDURE TO OBTAIN DEATH CERTIFICATES:
In cases when dead bodies are brought to Crematorium or Burial Ground under KMC for disposal -

Death events only in the KMC area are registered in the Crematorium or Burial Ground and death certificates are issued from there on the spot through Janma Mrityu Tathya Portal of Govt. of West Bengal. Certification of Death Event [Form 4/4A] must mandatorily be submitted to the Sub Registrar for issuing of Death Certificate.

2) In cases when dead bodies, death events of which have taken place in KMC area are disposed of in place outside KMC area-

Death events are registered in the KMC Borough where the event of death has occurred and death certificate can be obtained from there through Janma Mrityu Tathya Portal of Govt. of West Bengal on production of copy of certificate of death issued by institution or physician and original cremation certificate.

 
  IV. NON AVAILABILITY CERTIFICATE:
If an event of birth is found not registered, a Non-availability certificate can be obtained from KMC, Central Municipal Office Building, Health Department at 5, S.N. Banerjee Road, Kolkata - 700 013 on production of supporting documents like
A. Documentary evidence that denote that parents were residents of the city of Kolkata at the time of birth of the child.
B. Relevant corroboratory document/s of the child eg. Educational Certificate, Aadhar Card , Voter card , Khadhya Sathi ,
     Pan Card , Passport etc. to be also provided as linkage of relationship.
C. Satisfaction of the Registrar /Sub-Registrar on the specific issue.
 
  V. CHARGES PAYABLE FOR BIRTH AND DEATH CERTIFICATE:
1) For first copy of Birth Certificate within a period of one Year from the date of birth Free of cost
2) Non-Availability Certificate Rs. 500/-
